Laparoscopic cholecystectomy (LC) has become the customary method for treating gallstones worldwide, with certain events and complications appearing more frequently than with the open technique. Several aspects of these complications and their treatment possibilities have been analyzed, and clipless cholecystectomy with the harmonic scalpel is one of them. Over the past decade, laparoscopic cholecystectomy has become the procedure of choice for the surgical treatment of symptomatic gallstones worldwide. The operation is not completely risk-free, with some events and complications occurring more frequently than with open cholecystectomy. The use of the harmonic scalpel for cystic duct ligation is considered safe and comparable to clip placement at the surgeon's discretion. According to published articles by several authors, harmonic scissors are effective and safe in laparoscopic cholecystectomy as the sole instrument for sealing and dividing the artery and cystic duct. The main advantages may be related to safety and reduced operative time. Further research, including larger cohort studies and cost-effectiveness assessments, will further support the increased use of the harmonic scalpel in laparoscopic cholecystectomy.
